Please help Lost Dogs of Wisconsin know when your dog has been found and reunited with its family. This report can also be used when a loose dog that you have found has been reunited. This report should be filled out by the owner or Good Samaritan who picked up the dog.
You will be helping LDoW collect necessary data to share with those who have lost pets, with shelters, rescues, veterinarians, and organizations who will be seeking strategies to reunite pets with their families. We will never share your personal information.
